Explanation of fees

94FT Of Familiarity Skills Academy Monthly Training Fee’s – $125
All training fees are to be submitted by the 5th of each month and after the 8th a $25 late fee to be applied.

Practice Uniform – $75
*All members, new and returning, MUST wear a 94ft reversible jersey and shorts to every practice. No practice sets or uniforms will be ordered until full payment is received.

Financial Agreement

Dear 94FT Of Familiarity CSA Basketball Academy Parents.

Your commitment and support is the strength of our program and we thank you for it.

By working together, we will be able to help your student-athlete accomplish many of his/her goals both on and off the court. Our goal is to not only prepare each athlete for the upcoming seasons but to also prepare them for the ever-present and continuous aspects of life.

Training fees are $125/month
This fee includes four (4) 1-hour and 20-minute training sessions per month. Training fees are payable on the 5th day of each month and no later than the 8th of each month. A late fee of $25.00 will be assessed for payments received after the 10* of the month.

Participation in tournaments is charged separate from dues and is due 7 days prior to participation in the event. We now have a variety of payment options in place which include:

The Skills Academy is not a pay per sessions program. Payments are required in advance as I am contracted with the facility and have a monthly rental fee that must be paid on time*

*As a note, makeup sessions or missed Skills Academy sessions are announced. Beyond the makeup session dates, there will be no extensions into the next month. There are exceptions and I will handle those on a case by case basis.

By submitting the form above you agree to conform to the above payment schedule for participation in 94FT Of Familiarity CSA or O3 Elite YBA programs.

Waiver and Release Form

As the parent and/or legal guardian of the named player herein, acknowledges and understands that the sport of basketball is a contact sport wherein injuries may occur during the course of practices, scrimmages, and games. As such, the undersigned hereby voluntarily waives and release 94FT Of Familiarity CSA , Youth Basketball Academy, its Director, Trainer, coaches, and the applicable facility from any and all claims he/she may have for any and all injuries that the named player herein may sustain during the course of the player’s involvement and participation with 94FT Of Familiarity CSA, Youth Basketball Academy.

Medical Release Form

As a parent and/or guardian, I do herewith authorize the treatment by a qualified and licensed medical doctor of the following person in the event of a medical emergency which, in the opinion of the attending physician, may endanger his/her life, cause disfigurement, physical impairment or undue discomfort if delayed. I also authorize 94FT Of Familiarity CSA/O3 Elite YBA staff to remain with my child in such case of an emergency and for any/all medical information to be disclosed a 94FT Of Familiarity CSA, O3 Elite YBA staff member. In case of a minor, this authority is granted only after a reasonable effort has been made to reach the parent and/or guardian.

Media Release Form

94FT Of Familiarity CSA Youth Basketball Association personnel and members of the media may ask to interview, videotape, and/or photograph student-athletes of 94FT Of Familiarity CSA/O3 Elite YBA. Pictures, videotape, and interview content received, may be posted on the 94FT Of Familiarity CSA, O3 Elite YBA website, as well as other club websites and various social media platforms.

Please indicate your agreement for your child to be photographed, interviewed, or videotaped by completing the form below. This form also gives permission for such photographs, videotapes, and information to be posted on the 94FT Of Familiarity CSA website, as well as other club websites. Such permission, as is granted below, will remain in effect until revoked, in a written document, delivered to 94FT Of Familiarity CSA, O3 Elite YBA.

I agree to allow my child to participate in these activities, and for the resulting materials to be used, without financial remuneration, and I understand that this form completely releases 94FT Of Familiarity CSA, O3 Elite YBA from any and all liability, claims, or demands for remuneration or damages, arising from the use of said materials.
